چکیده

2015 Intramural hematomas are defined as an accumulation of blood in the media with entry and exit points that may or may not be identifiable. Intramural hematomas can occur during percutaneous coronary interventions and have been reported to occur spontaneously. While a majority of intramural hematomas are identifiable on coronary angiography, intravascular ultrasound should be considered the gold standard for diagnosis given its superior imaging capabilities. Outcomes are variable and there are no guidelines for the treatment of intramural hematomas. Therefore, treatment of intramural hematomas should be based on patient characteristics and clinical scenario.

Incidence, morphology, angiographic findings, and outcomes of intramural hematomas after percutaneous coronary interventions: an intravascular ultrasound study.

BACKGROUND Intramural hematomas during percutaneous coronary intervention (PCI) have not been well studied. METHODS AND RESULTS We used intravascular ultrasound to determine the incidence, morphology, and clinical features of post-PCI intramural hematomas. In 905 patients with 1025 consecutive native coronary artery, non-in-stent restenosis lesions undergoing PCI, 72 hematomas were detected i...

Role of grey-scale intravascular ultrasound and ChromaFlo in deciding on treatment approach for spontaneous coronary dissection in a young woman.

Spontaneous coronary artery dissection (SCAD) is a rare cause of acute coronary syndromes. We present the case of a young woman with an acute coronary syndrome caused by SCAD, where grey-scale intravascular ultrasound (IVUS) and ChromaFlo were instrumental in deciding against interventional treatment. The patient's urgent angiogram gave the impression of a spiral dissection in the right coronar...

Extension of a Coronary Intramural Hematoma as a Complication of Early Percutaneous Coronary Intervention after Thrombolytic Therapy

The optimal treatment approach for coronary intramural hematomas has not been well defined, and discussion is limited to scarce data. In addition, the impact of prior thrombolytic therapy in the setting of coronary artery dissections with possible development and/or extension of an intramural hematoma is not well understood. We describe a case of iatrogenic periprocedural dissection of the left...

A Case of Intra- and Extra-Mural Hematomas During Recanalization for Chronic Total Occlusion

An intramural hematoma is an accumulation of blood between the internal and external elastic membranes within the medial space, whereas an extramural hematoma is a dilution and/or dissemination of blood throughout the adventitia. Intra- and extra-hematomas are observed by intravascular ultrasound during percutaneous coronary intervention (PCI). The patient described herein presented with angina...

Diagnosis of intramural coronary artery in transposition of the great arteries using two-dimensional echocardiography.

BACKGROUND An intramural coronary is an uncommon but potentially significant risk factor for transfer of the coronary arteries as part of the arterial switch operation for transposition of the great arteries. Preoperative diagnosis is advantageous because it helps prevent accidental injury to the intramural coronary artery during transection of the aortic root and excision of the coronary arter...
